At Your In-Home Rehab, we welcome referrals from general practitioners, medical specialists, hospitals, case managers, support coordinators, aged care providers, and private health insurers. We support clients accessing Medicare, Home Care Packages (Support at Home), the NDIS, workers' compensation, and private health insurance, assisting with referrals, claims, and rebates to help make the process as simple and seamless as possible.

Assessment, treatment, rehabilitation, and return-to-function services for clients recovering from work-related or motor vehicle injuries.
Private Clients
Flexible home-based allied health services for clients who prefer to self-fund their treatment without requiring a referral or funding program.
Motor Vehicle Accident Schemes (CTP)
We support clients recovering from motor vehicle accidents through approved CTP insurance claims, helping restore mobility, function, and independence.

2 - Attach any relevant patient files or treatment notes to help us understand the patient’s specific needs.
3 - Our team will contact the patient promptly after receiving the referral to discuss next steps and support the transition into rehabilitation.
